For your convenience, we have added downloadable patient forms you can print out at home or the office and fill out prior to your scheduled appointment. Please bring the completed forms with you to your appointment. Our doctors also ask each patient to bring the following:

• Any eye glasses you currently wear
• Back-up glasses for contact lens wearers
• Any prescription or non-prescription sunglasses
